Usually it's grandmas and great aunts and various other fuddy-duds who tell you this nowadays. It might be used by an older generation, but it's definitely still got the same meaning behind it. It's saying to be quiet and keep a secret. It's like saying "keep it on the DL" but for your parents' gen.
