The dead man insists on the metaphorical and life-long nature of his suffering. Even if he actually drowned in cold water, that's kind of beside the point. What he really wants to get across is that he always lacked the warmth of companionship. His greatest suffering was mental and emotional. Add to this the fact that he's been misunderstood in death as in life, and that's quite a recipe for post-mortem pain.
