Drug Addiction and Dependence
If we had to pick a human antagonist, it would be the terrible doctor who prescribes Sara diet pills without looking at her and failing to monitor her progress on the drugs. But he isn't the real villain. He's an enabler of Sara's addictive tendencies.
Obsession, addiction, and dependence are antagonists for all our main characters, every though all of them have different addictions.
Sara and Marion
A lot of men want a girlfriend like their mother, and Marion and Sara are more alike than you might think. Both of them think more about themselves than Harry. Both of them end up looking happy (compared to the men) at the end of the movie. And Harry has trouble communicating with both of them.
It's no coincidence that in Harry's vision, Marion is wearing a red dress. Marion never wears a red dress in real life, only Sara does. These two woman blend together in Harry's dream, but he can't connect with either of them.
