You'll have to graduate tops in your class from a top school and get residency at a top hospital, where you make the kinds of connections and schmooze the right kinds of people to get you sitting pretty in the 90210.
You will intern with one of the big name surgeons to the stars, paying your dues and biding your time until either the practice is handed over to you, or the big boss is forced to step down, thanks to that nervous breakdown he has from working so hard.
If you kiss as many tuchases as you tighten, eventually someone will probably give you a break and let you into the club of powerful face fixers.
