Karl and Charles Bushman

Before we get to know Karl and understand why he's in a mental hospital, we see him in relation to a fellow patient: Charles Bushman. Bushman says disgusting things about women, violence, and murder. Karl reacts silently but with a look of distaste on his face. Karl's solemn reaction tells us a lot about him: he may be a murderer, but he's a murderer with ethics.
